       *   Copy of a physical dated within one year of 2014-2015 school starting date: August 19, 2014 

       *   Immunization Record

       *   Social Security Card

       *   Completed 2014-2015 Enrollment Packet (If needed internet access and assistance will be available at SPS)

After getting all of the above information,

Bullying will not be tolerated in Suwannee County Schools
It is important that our students have a safe learning environment and therefore there are several methods for reporting suspected bullying incidents:

* You can contact SPS by phone (386-647-4300) and report it to school personnel
* You can make a report in person
* You can submit an Online Bullying Report by going to the link below:

Title I/PALS Summer Tutorial/Recreational Program
Beginning June 16, 2014 until July 31, 2014
Monday thru Thursday, 8:00 a.m. - 1:00 p.m.
Ages 6-12 (Limited to 60 participants)
Cost: $10.00 P.A.L. Registration fee plus activities expenses.

Early Release Dates
The following dates are designated as early release days for Suwannee District School students. Unless otherwise indicated, students will be released from Suwannee Primary School at 12:40 PM on Early Release days.
September 3rd
October 1st
November 5th
December 3rd
December 19th
February 4th
March 4th
May 6th
June 4th
June 5th

Please make note the last two days of school will be EARLY RELEASE!!!!!  12:40

Wrist band reminders are put on students wrists the day before each Early Release.
Notification comes in newsletters sent home on Friday of each week.
Also, the Phone Master system will call your home with reminders.
Parents:  Be sure to bring your picture ID when checking students out from the office.
Also, we DO NOT take transportation changes over the phone.  Please be sure to send a signed note with your child when you need to change their transportation.
